Gross anatomy. The portal vein usually measures approximately 8 cm in length in adults with a maximum diameter of 13 mm. It originates posterior to the neck of the pancreas where it is classically formed by the union of the superior mesenteric and splenic veins (the portovenous confluence). The origin of the vein defines the location of the pancreatic neck.
