03.06.2013 · If tolerated, the dose is increased stepwise up to a maximum of 25 mg b.i.d. (50 mg in patients weighing 85 kg). Titration should be done slowly, increasing the dose at intervals of 1-2 weeks. The dose should not be increased in patients developing symptoms or with a systolic blood pressure 90 mmHg or a heart rate 50 beats per min.
