Portal venous gas (PVG) is a rare condition that can occur after blunt abdominal trauma. The case of a young boy with PVG after a car crash is reported. The boy died after surgery because of major posttraumatic bowel necrosis. The aim of this article is to show that PVG can be a serious condition after blunt abdominal trauma in children.

It was first described as hepatic portal venous gas (HPVG) in 1978 by Libman, et al.[2] Until 1968, HPVG was viewed as an inevitable harbinger of death with a mortality rate of over 90%. Libman, et al.[2] reported 64 cases of HPVG with an overall mortality rate as high as 75%.
